Abbreviated Oral Presentations

This format is useful for the “second H&P” presentation – typically done by the morning physician taking over care from the night float who admitted the patient (the “first H&P”):

  1. One- to three-line summary of patient
  2. Working diagnosis (admitting diagnosis)
  3. Interval data
  4. Hypothesis confirm/revision (with lovingkindness for admitting doc!)
  5. Treatment changes
  6. Care coordination (dispo, social challenges, etc.)
  7. Systems factors (both positive and negative)
